Therapeutic Communication In The Nursing Profession

Nursing is a caring profession.  It is also a profession that is more and more evidenced based in practice.  In
as much as the scientific aspects of nursing is increasing due to the
complex technological advancement of medicine and the machinery that is
used at the patients bedside, the fact remains that the nurse is the
first person that the client usually comes in contact with in any
emergency or hospital setting.   

Having said this,
the term, “caring” is an essential emotion that all nurses, for that
matter, all individuals in the health profession must possess.  With caring comes the trained ability of the nurse to facilitate therapeutic communication.  One might ask, what is therapeutic communication?    To better answer this question, the term communication should first be defined.  
Communication can be defined as “The Process of transmitting messages and interpreting meaning.” (Wilson and others, 1995)  With
therapeutic communication, the sender, or nurse seeks to illicit a
response from the receiver, the patient that is beneficial to the
patients mental and physical health.  Just as stress has
been proven to adversely affect the health of individuals, the
therapeutic approach to communication can actually help.  In any given situation everyone uses communication.  
 Everyone has seen the individual that looks like they are either angry, stressed, feeling ill or maybe sad.  These emotions are communicated to others not always by words, but by gestures and facial expressions.  A
nurse must always be aware of these expressions in clients, for these
expressions may be the only way that the nurse can tell if there is
something else going on that needs their attention.  The term given to this type of non-verbal communication is called, meta-communication.  In
meta-communication, the client may look at their amputated stump and
say that it doesn’t really look that bad, while at the same time tears
are rolling down from their eyes.  
 In a case such as this the nurse should stay and further explore how the person actually feels.  There are many factors associated with the healing and comforting aspects of therapeutic communication.  Circumstances, surroundings, and timing all play a role in the effect of therapeutic communication.  If
a client is being rushed down for an emergency surgery there might not
be time for a bedside conversation, but the holding of a hand could
convey much more than words to the client at such a moment.   
Ideally, for therapeutic communication to be effective the nurse must be aware of how they appear to the client.  If
a nurse appears rushed, for example, they are speaking quickly, their
countenance looks harried, and they are breathing heavily, their eyes
not on the client but perhaps on an intravenous bag on the client in
the next bed.  In a case like this, there is nothing that
this nurse could say to the client in a therapeutic manner that the
client would believe.  The helping relationship has not been established and therefore therapeutic communication cannot be facilitated.  Some
of the emotions associated with therapeutic communication include but
are not limited to the following: Professionalism, Confidentiality,
Courtesy, Trust, Availability, Empathy, and Sympathy. (Potter, Patricia

All of these emotions go into the client nurse
relationship, which must be established by the nurse as soon as
possible upon first meeting the client.  To begin to
establish this nurse client relationship, the nurse must assess the
overall message that the client is communicating to the nurse, such as
fear, pain, sadness, anxiety or apathy.  The nurse should be trained in keying into the message that the client is sending.  Only then can the nurse determine the best therapeutic approach.  Anyone that has to be thrust in to a hospital or emergency room environment has level of anxiety.   
This
level can go up considerably when the client feels that they have been
abandoned or that there is no one there that really cares about how
they feel.  When a client is the recipient of therapeutic
communication from a caring individual, a level of trust is achieved
and more than, that the clients entire countenance can change for the

The Truth About Per Diem Nursing

There are many great aspects to a career in nursing. You get
to help people, you get to meet people, and since nurses are needed across the United
States , you have the opportunity to work just
about anywhere you want.

But one oft-overlooked advantage of nursing careers is the
opportunity to schedule your career around your life. Thats exactly what per
diem nursing provides. If you want or need to schedule your own working hours,
but still earn a good salary, then this type of non-contract job may be the
ideal career choice for you.

With daily nurse jobs there is an opportunity to partner
with medical staffing agencies and work in temporary assignments at hospitals
and clinics. RNs that sign on for these nursing positions may sometimes work
on-call and will typically be scheduled week to week. A per diem nurse will
also be responsible for any last-minute requests to fill in for sick hospital
staff or for those who simply cant make it to work. In some cases, someone
with a per diem nursing position wont be called in until the day he or she is

preferred specialty rolesas a critical-care nurse, for example, or they can
choose to work in more general per diem nursing roles.

Per diem nursing is often described as shopping for nursing
shifts. Those who work on a daily basis can choose to work shifts that are as
regular or as irregular as they like. For those that love to travel, that can
be a big plus. By being able to set their own hours, the weeks that they are
available to work, per diem nurses can plan for short or extended vacations far
more easily than those working on a regular schedule.

Per diem nursing offers a taste of adventure that may not be
available to nurses with full-time jobs at a hospital or clinic. Its a good
resource for nurses who havent quite found their niche or specialty area. The
opportunity to work in clinics and hospitals of all different sizes and the
opportunity to discover what its like in new departments makes finding that
right nursing specialty easier than ever.

Others who can benefit from per diem nursing careers include
new moms and nurses close to retirement. Per diem nursing is also an excellent
choice for nurses who are taking continuing education classes and other nurses
who dont want to work full time, but want to work intermittent shifts in order
to keep their experience current. In some cases, per diem nursing gives nurses the
chance to test a hospital in order to get a feel for the work culture and
environment before signing on for a permanent position.

While per diem nursing can be a good part-time job and a
career booster for a wide range of people, its not for everyone. Taking a per
diem job means nurses may be scheduled to work in several different hospitals

Nurse mare foals are unfortunate foals born to mares that are bred solely for milk production. Nurse mare farmers use mares of varying breeds but often times the larger breeds, such as drafts, to breed nurse mare foals.

After foaling, the breeders lease the mares out to a farm, usually a farm involved with the horse racing industry. The mares are taken from their biological offspring anywhere between 1 day and 1 week of age. These new mothers then become mothers for more “expensive” foals. Their biological foals are left to die, are killed or are slaughtered due to the level care they require and the costs associated to raise the orphan foals..

Often while the mare is being leased out, the “expensive foals” farm is required to get the mare back in foal. This explains the mixed breeds of these foals and why they’re unable to be registered. The farmers will just breed back to whatever is available, even the teaser stallions, it doesn’t matter. This creates a vicious, but convenient, cycle for the nurse mare farmers, who make a profit on leasing the mare, and then get the mare bred back for free. This in turn brings in the same money the next year.

The Standards of Professional Performance For Nursing

All professions have as their cornerstone and creed a code of ethics and or professional performance standards. The nursing profession is no different. As the field of nursing grows and changes in defining the role of a nurse, the central theme will always be that of caring. It can be said that, all other duties and responsibilities of nurses have centered around, “caring”. Caring has been described by many nursing theorist as the innate nature of a human being to be able to give unconditional help in the form of doing for, therapeutic communication, therapeutic touch, compassion, a gentleness of spirit and humbleness of knowing ones self.

Jean Watson, a Ph.D. in Educational Psychology and Counseling developed a theory of caring first published in 1979. She believed that caring was central to the profession of nursing as well as being the unifying principle of practice. The two major assumptions that she made are as follows:

1. Care and love constitute the primal and universal psychic energy.

2. Care and love are requisite for our survival and the nourishment of humanity.

Madeleine Leininger, a nurse and anthropologist proposed a theory of caring based on culture. She was able to see that how one society defines caring is not always the same as another society. For instance, in Asia, looking directly into another’s eyes is not a sign of respect but a sign of disrespect. Therefore a western nurse unaware of the Asian culture of caring would be seen as an uncaring and disrespectful person. Leininger termed this knowledge of cultural caring as, “transcultural nursing”.

“Her beliefs are that culture is the broadest and the most holistic way to conceptualize, understand, and be effective with people.” (1)

With the emergence of the, “caring”, theorists also came the emergence of “Standards of Profession Performance”. The two terms are entwined in their definitions. There cannot be one without the other. The American Nursing Association set forth the following requisites for standards of professional performance. It is within these standards that can be seen the unifying theme of caring.

Standard 1. Quality of Care.

The nurse systematically evaluates and effectiveness of nursing practice. The nurse does this by participating in peer review committees and in consistently evaluating his/her own performance and how her performance has enhanced the clients health and emotional wellness.

Standard II. Performance Appraisal

The nurse evaluates his/her own nursing practice in relation to the national and state statues set forth defining the role of nurse.

Standard III. Education

The nurse acquires and maintains current knowledge in nursing practice. Many states now require continuing educational credits to be earned by a nurse. The number of credits vary from state to state. However, a nurse cannot renew their license without proof of these continuing educational credits. These credits help to keep the nurse current with new medica/nursingl advancements related to patient care.

Standard IV. Collegiality

The nurse interacts with and contributes to the nursing profession by meeting with other professionals in the medical field, such as Physicians, Physical Therapist, Respiratory Therapists, and Home Care Coordinators, to exchange ideas and gain a mutual respect while stimulating a leaning environment that all will benefit.

Standard V. Ethics

The nurse’s decisions and actions in helping and caring for individuals are based on principles of ethics and institutional guidelines.

Standard VI. Collaboration

The nurse collaborates with other medical professionals in creating an environment for the clients that facilitate an increasing continuum of mental and physical wellness.

Standard VII. Research

The nurse will not cease to expand his/her educational horizons, and continue to seek out and explore new areas of research and statistics.

Standard VIII. Resource Utilization

The nurse will participate in committees within the organization to evaluate and increase the effectiveness of safety, costs in planning and delivering patient care.

It can be seen that within the guidelines of professional performance the center theme is the caring for the client from every aspect. It is within these guidelines that the nurse can better define his/her role as a the one individual who has the ability to enhance the clients mental and physical wellness by coordinating, educating, and working with others to promote a caring and supportive environment.

The Salem Witchcraft Trial Connection

Serving as a black mark on the history of Massachusetts were the Witchcraft trials of 1692-1693. Rebeca Nurse and Sarah Cloyce were victims of the witchcraft frenzy.

Research has shown that a deep piety in this theocratic society intertwined with superstition, hysteria, fear and believe it or not economics all played a part in perpetuating this persecution and tragedy. A minister with limited education and after a year’s deliberation was ired by a church committee, his name was Reverend Mr. Parris. Most of the upper economics liked ministers with Harvard degree’s. His economic demands upon hire brought about a great deal of contention between the church members. He insisted that the church gifted him a parsonage and two acres of land. This was against the laws at the time. The Nurse family along with others, strongly opposed his views.

The Revered had also brought with him two Indian servants from Barbados where he had taken a year off from religion to trade in the colony of Barbados. Their names were Tituba and Indian Jones who would tell group of young women about conjuring spirits and holding ‘pow wows’. The Puritan girls bored with their own life enjoyed the tales that were considered extremely sinful pagan practice.

It was not long after that the servants were caught in what was called practicing incantations. The local physician claimed that they were bewitched. Two of the Puritian girls began to have odd experiences that they claimed were fit, seeing specters and having hallucinations. The physician was called and declared that Tituba was the cause. Tituba named Sarah Osburne (who was bed ridden) and Sarah Good (a social outcast) as her counterparts in bewitching the Puritan girls. All three were held for trial and accused of witchcraft. That was only the beginning of what would become more than a simple line in a history book.

After some time, the Reverend began to claim that every incident of children “crying out” against the villagers was an act of girls being bewitched inciting the congregation from the pulpit. The Norse family, the Cloyce family and Joseph Putnam became so utterly disgusted with the Reverend and his pulpit antics that they stopped attending the Church all together. As in most disputes that may outwardly seem to be religious in motive, economics lay just beneath the surface.

There was a boundary dispute that was caused by an unintentional error by the Court house resulting in Villages of Beverly, Topsfield and Salem having to pay town and parish taxes to Tospfield. This land dispute would not be settled for over a century. Disputes and hard feelings developed between the Putnam’s on one side and the Easty’s and Townes on the other side. Rebeca was a Towne and the family sided with Topsfield. This angered the Putnam’s and the hostility only escalated when boundary lines between the Nurses and the Putnam’s farms arose. The Nurses had made a name for themselves economically and resided in one of the finest house in the parish that was set on 300 acres of land. They were considered to be a family of untainted character and moral people that lived by the teachings of the Church.

So when the Putnam’s were the complainants on the arrest warrant for Rebeca Nurse, many were appalled. On March 23, 1692 a warrant was issued for the indictment of acts of witchcraft against Ann Putnam and unnamed others. At this time Rebeca was 71 years old, deaf, weak, bedridden and ill. Still she was arrested and taken to jail to await trial.

John Proctor was one of the few that stood up for Rebeca and her character. When his own servant Mary Warren also accused Rebeca, Mr. Proctor threatened her with a whip if she even moved from her spinning wheel. Until the day that Mary was called to testify against Rebeca there was not a single episode of fits. However, upon the stand miraculously the fits returned. On June 30 Rebeca appeared dressed in a proper gray gown, white collar and white-winged Puritan cap. Abigail Williams testified that she had been bewitched by Rebeca. Her reply was “I am innocent, and God will clear my innocence.”

Ann Putnam’s mother went on to claim that the spectral shape caused by Rebeca’s bewitching caused her to do evil and further claimed that Rebeca was what caused the deaths of Ann’s young children. The Court had decided that for these particular cases to allow spectral evidence. What this really meant that dreams, hallucinations and fancied imaginations would be accepted as factual proof. How does one defend themselves against ‘you were in my dreams last night’.

Rebeca was convicted and taken to Salem jail. Sarah Cloyce was so angry and outraged that during a pulpit sermon ranting about witchcraft and insisting every witch should be destroyed, that she got up in the middle of the sermon and walked out of the church ensuring the door slammed behind her.

At the trial Rebeca was found guilty and sentenced to death. On July 3, she was officially excommunicated from her church. The men of the Nurse family got a petition together and went to the Governor. The Governor granted a reprieve being displeased with the judge’s proceedings. The news of his reprieve of Rebeca caused such an outrage that the Governor feared of being deposed and rescinded his order. So on July 19, 1692 Rebeca Nurse at the age of 71 was hung.

On April 4, while the Rebeca case was going on, a complaint was made against Sarah Cloyce who was Rebecca’s sister. A warrant was issued on April 8, followed by a trial on April 11. The servant Indian John testified that Sarah had chocked him and made him sign the Devil’s book. Indian John was afraid of also being accused of being a witch as Tituba had been and decided it would be better to be one that had been bewitched. During the trial Mary Wolcott and Abigail Williams accused Rebeca, Sarah Good, Martha Corey and Sarah Cloyce had all been seen at witches meetings. At the same time Elizabeth and John Proctor were also tried and were remanded to a jail in Boston.

Sarah Cloyce was later transferred to confinement of her home, her husband Peter moved her to Framingham where she stayed until her death in 1703. Many were later released to the confinement of their homes never seeing the streets of Salem again.

The Responsibilities Of A Nurse Been Changed

Isn’t it the truth that people who are interested in sending out special help they are interested in the career of nursing? The truth is that the Dean of the School of Nursing of the University of Rochester has brought it to everyone’s attention that the rewards involved in this career have raised. However when it comes to academics no one can say anything. Prior to this she spent many years as a nurse herself.

There may be times when she feels that maybe she should have remained in the field of nursing and that taking up teaching wasn’t the right decision. The general responsibilities of a RN can be divided into four. The first is the nurse should observe and record the medical history of the patient. This is including the symptom that is faced by the patient. The next is to establish the care plan and carry it out to the action. The RN also has to manage the tasks for the nursing assistant and the licensed practical nurses. The last one is to provide the emotional supports for the patients and the families.

First thing you should do to become a RN is getting a BSN (4-year degree). By taking this degree, you will get the general and science education you will need later. The lesson is including the biology, anatomy, physiology, Standard English, Math, and also chemistry. You also need to take the NET or the Nursing Entrance Test. This test will help you to enter the core of the nursing means you will be able to start your nursing curriculum if you can pass this test. The BSN program itself will help you to learn the advanced medical subject related to the medicine and science areas.

You will finish the BSN program with clinical externship where you will work in the health care facilities for weeks or months. There will be another nurse or the doctor that will supervise you. You must do great job in this practice since it will be counted for your overall grade.
